We are using a new online Register with a GP surgery service that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
If you would like to register with the practice please click the New Patient Registration button below or the following link to print-out a form to fill in manually – Register with a GP surgery (print & complete)
We require your previous medical records before prescribing regular medication. Therefore please ensure you have a month’s supply of regular medication from your previous surgery before registering.
Please read our Data Sharing Information page before completing the form.
Please note that you will receive a welcome text from the surgery when your registration has been processed to advise you that you are now registered with the surgery.
The CCG has produced a guide to healthcare services in Devon for GPs to use with patients who are from an ethnic minority or who are new arrivals in Devon.
All Ukrainian arrivals are entitled to full and free access to the NHS. To help individuals register with GPs, the NHS have developed bilingual registration forms and is reminding parents that proof of address or other forms of identification are not required when registering.
