Register as a patient at Peverell Park Surgery

We are using a new online Register with a GP surgery service that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Important: We require your previous medical records before prescribing regular medication. Therefore please ensure you have a month’s supply of regular medication from your previous surgery before registering.

Named Accountable GP – From 1 April 2015, it has been a contractual requirement for all GP practices in England to assign every patient a named, accountable GP. This GP is responsible for overseeing your overall care and ensuring that appropriate services are provided.

What this means for you: 

  • You will still be able to see any GP or clinician in the practice—your care is not restricted to your named GP.
  • The named GP role is primarily for continuity and administrative purposes.
  • You have the right to express a preference for which GP you are assigned to. If you would like to change your named GP, please contact the practice and we will do our best to accommodate your request.

This policy helps maintain clear responsibility for patient care while giving you flexibility and choice.

Meeting the initial health needs of people arriving in the UK from Ukraine

All Ukrainian arrivals are entitled to full and free access to the NHS. To help individuals register with GPs, the NHS have developed bilingual registration forms and is reminding parents that proof of address or other forms of identification are not required when registering.
