New Patient Registration

Register as a patient at Peverell Park Surgery

We are only able to register patients within our Practice Boundary, check the NHS Find a GP service to see if you fall within our catchment area.

We are using a new online Register with a GP surgery service that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

If you would like to register with the practice please click the New Patient Registration button below or the following link to print-out a form to fill in manually – GMS1-family-doctor-services-registration-form

Important: We require your previous medical records before prescribing regular medication. Therefore please ensure you have a month’s supply of regular medication from your previous surgery before registering.
