We are encouraging patients with diagnosed or suspected high blood pressure to monitor at home and share their readings with us. 

Recording and Submitting Your Average Blood Pressure Readings

• Take two blood pressure readings each day, one in the morning and one in the evening, for four days
• Record your readings in a diary or notebook
• Once you have completed your paper diary, enter the readings into the form below to calculate your average
• The calculator will display your average systolic and diastolic blood pressure at the bottom of the form
• To submit your average blood pressure to the surgery, please enter the values in the text message we sent
