We understand that if you have a disability or a condition that makes things difficult for you, getting the right help from the NHS or social care might be challenging. However, making some changes to accommodate your needs can improve your experience. These changes are known as Reasonable Adjustments.
If you need reasonable adjustments, we really want to ensure we make those changes for you. To do this, we need to know what adjustments you might require, as these will vary from person to person.

⇒ Providing help for you to complete NHS forms
⇒ Making sure you have a quiet place to wait for your appointments
⇒ Adjusting appointments based on carer availability
