Reasonable Adjustments Digital Flag

We have recently been informed by the RADF National Project Team that recording Reasonable Adjustment Digital Flags is now mandatory. We have a legal obligation to ensure these flags are recorded in our clinical system when appropriate. The current deadline for this requirement is September 2026.

TPP is working with the RADF National Project Team to enable their API (interface) to go live. Once this occurs, the flags will be automatically uploaded from our system to the NHS Spine in a live environment, including any flags that have already been recorded. Read more….

DELT: How to Create Strong Passwords

Delt have put together some simple tips for creating strong and memorable passwords. For more guidance and examples, take a look at Tips for Remembering Passwords.

It is always a good idea to update your passwords regularly. Remember, they should be difficult for others to guess while still being easy for you to remember!

>> Use a memorable phrase – Turn a favourite sentence into a password by using the first letter of each word.
>> Combine two unrelated words – Blend them together and replace some letters with numbers or symbols.
>> Mix words and numbers – Alternate letters from a word you like with a number pattern that’s meaningful to you.
>> Create your own formula – Develop a simple pattern or structure you can apply to generate unique passwords for every account.

NHS 111 – Moving from signposting to digital referral

Since December 2025, the NHS 111 telephone service has begun electronically referring patients to a community pharmacy of their choice for additional Pharmacy First minor illness clinical condition groups. This will replace signposting for these conditions and will allow pharmacy teams to view these referrals directly in clinical systems.

These changes will increase system efficiency and enhance the patient journey. According to reports, it is estimated that approximately 75,000 annual signposts to community pharmacies will be converted to digital referrals. Online electronic referrals will follow in March 2026.

Pharmacy First digital referrals can come from:

>> NHS 111 Telephony
>> General Practice 
>> NHS provider urgent care Streaming and Redirection

DELT Case Study: Patient Consent

An elderly female patient with Type 2 diabetes, Chronic Obstructive Pulmonary Disease (COPD), and a recent diagnosis of early-stage dementia, lives independently in her own home. She attends the GP practice regularly with support from her adult daughter, who is listed as her next of kin but does not hold power of attorney. 
Recently, the patient has experienced several falls and missed medication doses. At the same time, the daughter has been frequently contacting the Practice asking for detailed information about her mother’s medical history, test results, and medication reviews. Read More…….
